Dear Parent/Guardians

EPIC is partnering with Planet Youth to better understand how we can help our young people grow up healthy, safe, and supported. We will be conducting an anonymous youth survey with students in the Estill County School District. This survey is part of the Icelandic Prevention Model, a proven approach that has helped communities worldwide reduce youth substance use and improve well-being.

According to Senate Bill 150, adopted by the Kentucky General Assembly in 2023, parents must opt in to any survey that asks students about sensitive information, even when the survey is anonymous. The data collected in this survey will be available to the public once the report is complete. You may view the blank survey that will be used in 2026, as well as the previous survey data collected in 2024, by scanning the QR code or visiting www.estill.org/EPIC

Quick Facts about the Survey

• Anonymous & Confidential – No names or personal identifiers will be collected.
• Topics Covered – Hobbies, school experience, family and friends, health and well-being, attitudes toward alcohol, tobacco, vaping, and other substances.
• Purpose – To guide programs, activities, and supports that benefit our youth.
• Voluntary – Students can skip any question or stop at any time.
